Sport Registration

In order to be eligible to play a NHIAA sanctioned sport, every athlete must complete the following steps. Student athletes must have all 4 pieces of the registration process completed before they will be allowed to participate in a tryout or practice.

  1. Physical Form.
    1. Physicals are good for 365 days and must be updated every year. Your physical on record cannot expire during your season of play and must be kept on file with the Athletic Department. Click here for a copy of this form to bring to your doctor.
      1. If your student-athlete's physical is expired, which means their last physical was before 8/13/17, and you have a new physical scheduled but it has not happened yet the school needs a copy of a note from your student-athlete's doctor stating they are approved for sports until their physical or your student-athlete will not be able to try out. The note may be faxed to the school or delivered in person on Monday by your student-athlete.
    2. If you need a physical and have been unable to get one scheduled before the season starts, click here for more information on sport physicals through Convenient MD
  2. Athletic Participation Permission Form
    1. This form helps us collect important information on health insurance, who to contact in an emergency, and other relevant medical information. Click here to access a copy of this form.
  3. Impact Test (high school only)
    1. This test is used by the school district to help evaluate head injuries should an athlete sustain one during a school sponsored event. The test is required for athletes every other year and helps us to establish a baseline for use if an injury occurs. Impact test are administered at the high school prior to the start of each season. For more information on the Impact Test and the science behind it, please click here.
    2. For more information on the Shaker Regional School District Concussion and Head Injury Policy, click here.
  4. Athletic Handbook
    1. Parents and student athletes must read through the BHS or BMS Student Athletic Handbook, then sign and return the last page to their coach. The handbooks contain a great deal of information on rules and expectations for athletes in the Shaker Regional School District. To download a copy of the handbook click here for BHS students and click here for BMS students.
